Dear Patients and Colleagues,

This is to advise and inform everyone that I am closing my sports medicine practice effective April 15, 2011. My family and I are moving out of the Southern California area and will no longer be able to provide primary care sports medicine services after April 15, 2011. However, I will be available until that time for patient care or any questions.

I recommend following up with the primary care physician of record after April 15, 2011; or for those needing specialty services, I have listed trusted colleague specialists under Resources. Please select another physician to continue your care after April 15, 2011. If you have a primary care physician, that individual will be an excellent resource for care or future referrals.

If you would like to find another primary care sports medicine physician in the area, I recommend searching www.amssm.org and clicking “Find-A-Doc” for a searchable database.

I have enjoyed being your primary care sports medicine and musculoskeletal specialist in the Burbank / Glendale area since 2004.

Sincerely,

Gary Arends, Jr, D.O., APC
